Health Care Partners Foundation is a non-profit organization that aims to improve the well-being of vulnerable populations in our communities.
We utilize Telehealth connectivity to provide care coordination for medical and mental health services through a network of hospitals, community agencies, and residents. Our goal is to create cost-effective, integrated systems that offer 24/7 care coordination triage and crisis intervention, telehealth services, medication assistance, and online patient education.
We believe in forming partnerships to improve the delivery of health care services to high-risk populations, with a focus on individuals with mental health or substance abuse disorders. Our vision also aims to increase collaboration among all major network partners.
